血管性痴呆和帕金森病痴呆患者听觉认知电位的比较

【摘要】目的:探讨听觉事件相关电位在血管性痴呆、帕金森病痴呆中的差异及其意义。方法:应用事件相关电位检测方法对1999-01/2004-12汕头大学第一附属医院神经内科住院的22例血管性痴呆患者和18例帕金森病痴呆患者及26名正常人进行检测,并进行对比观察。电极按国际标准导联10~20系统置于T5,T6,P3,P4,Cz,Pz,T3,T4,C3,C4,F7,F8,F3,F4,Fz,Fpz,参考电极为A1+A2,地线置于Fp1,低频滤波0.5Hz,高频滤波30Hz,分析时间700ms。采用听觉oddball序列,通过耳机给声,1000Hz为非靶刺激,概率80%;2000Hz为靶刺激,概率20%。刺激间隔1s。对靶和非靶刺激分别叠加。每例检测重复两轮,以确认波形成分重复性好。潜伏期及波幅以Pz点测得的为准进行分析。组间比较采用SPSS10.0软件包处理。结果:各组均获各项检查指标,全部进入结果分析。①事件相关电位的内源成分:P300电位(即P3波)在各组间异常程度不同。与对照组比较,血管性痴呆组P3潜伏期最长,帕金森病痴呆组次之犤(415.5±48.5),(345.3±34.1),(317.9±26.9)ms,P<0.05犦。②事件相关电位的外源成分:各组N1波改变各异,血管性痴呆组与对照组相比潜伏期延长犤(108.2±25.2),(96.4±10.6)ms,P<0.05犦。波幅降低犤(5.1±3.4),(8.5±3.2)μV,P<0.05犦,帕金森病痴呆组潜伏期及波幅均正常。③峰间期:血管性痴呆组与对照组比较P2-N2、N2-P3、N1-P3峰间期延长,而帕金森病痴呆组各波峰间期正常。结论:血管性痴呆及帕金森病痴呆两种疾病听觉事件相关电位异常程度及异常成分存在差异,其电生理特征对区分痴呆类型具有一定价值。
